The Governor's Office of Economic Development would like to invite you to participate in a brief survey designed to understand the issues confronting Utah small business owners who would like to offer a health benefit to their employees.

As we approach Utah’s legislative session, we hope to find some solutions to the health care problem for small businesses in Utah. But to do this, we need your input. We know how busy you are, so the survey is comprised of 14 questions and should take 3-5 minutes. Because of the importance of this survey, we are using various mailing lists, and we apologize if you receive this more than once. If you have already completed this survey, thank you. You need not do so again.

Complete entries will be entered in a drawing to win a $75 gift certificate: dinner for two at Cafe Madrid. To participate in the drawing you must enter contact information at completion of survey.
